The Voucher Request Form

STEPS TO TAKE
1.  Complete the Registration Sign-up Form on this web site.
2.  Complete a Navy Voucher, which can be approved by anyone at the command who is senior to the person attending the symposium (E-6 or above).
3.  Submit that voucher to the Navy Voucher office - email or fax noted on the form.
4.  Get all this done at least 5 days prior to the class.
5.  If your EAOS is less than one year from the first date of the class, and you plan to re-enlist, the voucher office says, "We only need a "chit" stating the intent to re-enlist."
